Background: A health-promoting lifestyle is a key determinant of health in contemporary societies. However, the persistent gap between health knowledge and actual behavior remains a major challenge in health promotion. This study aimed to explain the relationship between health culture and a health-promoting lifestyle and to propose an integrated conceptual model. Methods: This theoretical review was conducted using a conceptual synthesis approach. Relevant literature on health culture, health-promoting lifestyle, health literacy, health habits, structuration theory, and the social determinants of health was critically reviewed and synthesized. Based on the extracted conceptual relationships, a conceptual model was developed. Results: The literature synthesis indicated that the relationship between health culture and a health-promoting lifestyle is neither direct nor linear but is mediated by two key mechanisms: health literacy and health habits. The proposed conceptual model comprises four interconnected levels: health culture, health literacy, health habits, and health-promoting lifestyle. The model further demonstrates that structural and social factors play a moderating role throughout this process. In addition, it highlights a feedback relationship between a health-promoting lifestyle and health culture, whereby health-promoting behaviors contribute to the reproduction and reinforcement of health culture. Conclusion: The proposed model provides a theoretical framework for explaining the development of a health-promoting lifestyle and suggests that promoting healthy behaviors requires simultaneous actions at the cultural, cognitive, behavioral, and structural levels. The model may serve as a foundation for future research and the development of health promotion programs.
