A Review of the Concepts of part 2 of the General Health Policy, announced by the Supreme Leader

Abstract
Background and Aim: General Health Policies announced by Islamic Republic of Iran`s supreme leader is legal ,mandatory, and definitive duty on the basis of shari'a and according to the constitution of the country.
 
The provisions of the document are comprehensive and complete and it covers health objectives in the I.R of Iran based on social justice, people's participation, intergovernmental cooperation, and the use of appropriate technology. Part 2 of the policies emphasize the issue of health all -round and healthy people. And the politician insists that all actions of the parliament and the government in the establishment of laws and regulations should be based on the provision and promotion of all four dimensions of health. So this study attempts to explain and localize the concepts and introduce the indicators and general interventions for implementation of the provisions.
Method: First, in order to accurately define the key concepts, internal and external documentation were reviewed. In the next step, using the content obtained in the first step  and the opinion of the experts in qualitative method stakeholders were identified and  a list of indicators and interventions for relevant subcategories were classified.
Results: In this study, 30 general concepts were identified and explained for each sub-sections. And 35 relevant indices and 51interventions were selected  in the form of six sub- sections.
Conclusion: Health in all policies reflects a multidisciplinary plan for national or state health policy. Health in all policies has increasingly been highlighted as a government strategy to improve the health of the community through coordinated action across health and non-health sectors. Health determinants are the core of health in all policies. The effective ways to maintain health and prevent diseases and threats in the community and facilitate the achievement of Iran's goals of 1404  are  prioritizing prevention, observation of environmental and social changes, and correction of protective measures as well as reviewing and updating current health plans in the country, strengthening monitoring and evaluation systems in programs and evaluations for standards in developing the country's policies and strategies. There for, in first step country laws and major policies in a variety of areas should be health-oriented. The next step is implementing the rules and regulations, in compliance with principles of health and the values of the community.
In order to plan and implement part 2 of the policy, there are significant requirements in the country. For example, the past experience of establishing an efficient and justice-based network for providing all primary health care and a successful model for training human resources and integrating medical education, as well as support legislations , such as Article 89 of Iran 6th Development Plan, which obliges the Ministry of Health to implement a comprehensive service system and public health, the priority of primary health care, and establishment of primary care referral system in family practice, the level of service, the Iranians electronic health record, or the National Health Standards Act, approved by the High Council on Health in June 2012, and Plan and Budget Organization has been communicated to the executive agencies, but has not implemented yet.
One of the key elements for implementing part 2 is to clarify the stewardship of health system and to establish this concept Ministry of Health and Medical Education is responsible for this function, as clearly stated in part 7. The Ministry of Health is responsible for policy making, planning, monitoring and evaluation but it has not used authority well. It seems that this belief has not been well established in other executive agencies such as the Ministry of Cooperatives Labor and Social Welfare, Ministry of Industry, Mining and Trade and Ministry of Agriculture -Jahad.
In order to create plan to achieve this part goals (like other parts), it is necessary to explain the image of healthy society firstly and then reveal the health system`s function in primary, secondary and tertiary preventive care and their quality of care.
