An Analysis of Challenges and Strategic Recommendations for Implementing the Family Medicine Program in Iran: Insights from the Fars and Mazandaran Provinces

This policy brief analyses the experience of implementing the Urban Family Physician Program in Fars and Mazandaran provinces since 2012. Drawing on interviews with 12 experts and questionnaires distributed among policymakers, managers, service providers, and health researchers, the brief applies a SWOT framework to examine the strengths, weaknesses, opportunities, and threats of the program. Findings indicate that nationwide implementation requires structural reforms in three key areas: financing, payment systems, and contracts/regulations. Sustainable financing through insurance funds, fair tariff-setting and infrastructure for performance-based payments, as well as contracts that incentivize physician retention and service quality, are identified as critical prerequisites for the program’s success at the national level. Furthermore, political commitment, public awareness, alignment of medical education with program goals, and active community participation are essential to ensure the program’s sustainability and effectiveness. |
