fa ارزیابی هزینه- فایده مداخلات مشاوره‌ای داروساز در بخش مراقبت‌های ویژه تخصصي Special اصيل Original Article <div style="text-align: justify;"><span style="font-size:8pt"><span style="line-height:18pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span style="letter-spacing:-0.15pt"><b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">زمینه و هدف : </span></span></b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">بیماران بخش مراقبت‌های ویژه نیاز به بررسی و مراقبت بیشتر دارند. حضور داروسازان برای دارودرمانی این بیماران در سال‌های اخیر بسیار مورد توجه قرار گرفته است و تأثیر مثبت حضور آنها دیده شده است.<b> </b>هدف از این مطالعه، بررسی تأثیر مداخلات دارو‌درمانی داروسازان بالینی در بخش مراقبت‌های ویژه بود.</span></span></span></span></span></span></span></span><br> <span style="font-size:8pt"><span style="line-height:18pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span style="letter-spacing:-0.15pt"><b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">روش: </span></span></b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">این مطالعه به صورت تحلیلی- مقطعی و آینده‌نگر در پنج بیمارستان شهر تهران به مدت 7 ماه انجام شد. اطلاعات دموگرافیک بیماران جمع آوری و مداخلات داروساز بالینی و انواع آن بررسی شدند. تأثیر این مداخلات بر کاهش هزینه‌ها از طریق تأثیر مستقیم بر تجویز بهینه دارو‌ها و جلوگیری از هزینه ناشی از عوارض دارویی به دست آمد. سپس سود حاصل از این روش با استفاده از تفاوت میزان صرفه‌جویی در هزینه‌ها و حقوق پرداختی به داروساز بالینی محاسبه شد.</span></span></span></span></span></span></span></span><br> <span style="font-size:8pt"><span style="line-height:18pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span style="letter-spacing:-0.15pt"><b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">یافته‌ها:</span></span></b><b> </b><b> </b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">در مجموع 350 مداخله بر روی 137 بیمار انجام شد و 87 مداخله باعث کاهش میزان بروز عوارض دارویی شدند. حضور داروساز بالینی در بخش مراقبت‌های ویژه، از نظر اقتصادی منجر به منفعت خالص 17-10 میلیارد ریالی در طول این مدت شد.</span></span></span></span></span></span></span></span><br> <span style="font-size:8pt"><span style="line-height:18pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span style="letter-spacing:-0.15pt"><b><span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">نتیجه‌گیری:</span></span></b> <span lang="FA" style="font-size:11.0pt"><span b="" nazanin="" style="font-family:">مداخلات داروساز بالینی در بخش مراقبت‌های ویژه نه تنها به بهبود نتایج بالینی بیماران کمک می‌کند؛ بلکه می‌تواند به کاهش هزینه‌های درمانی و افزایش کیفیت خدمات ارائه‌شده نیز منجر شود. با توجه به افزایش پیچیدگی درمان‌های دارویی و موارد بالینی، توصیه می‌شود که برنامه‌های آموزشی و مداخلات داروسازان بالینی به‌طور جدی در نظر گرفته شوند تا بتوان از تمامی مزایای بالقوه آنها بهره‌مند شد.</span></span></span></span></span></span></span></span></div> <div style="text-align: justify;"><span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:12.0pt"><span style="page-break-after:auto"><span style="unicode-bidi:embed"><span style="font-size:10.0pt">Background</span><span style="font-size:10.0pt"><span style="font-weight:normal">: Patients in the intensive care unit need more investigation and care. The presence of pharmacists for the drug treatment of these patients has received much attention in recent years and the positive effect of their presence has been seen. The aim of this study was to investigate the effect of drug therapy interventions by clinical pharmacists in the intensive care unit.</span></span></span></span></span></span></span><br> <span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:12.0pt"><span style="page-break-after:auto"><span style="unicode-bidi:embed"><span style="font-size:10.0pt">Methods</span><span style="font-size:10.0pt"><span style="font-weight:normal">: This study was conducted in an analytical-cross-sectional and prospective manner in five hospitals in Tehran for 7 months. Patient demographic information was collected and clinical pharmacist interventions and their types were analyzed and categorized. The impact of these interventions on cost reduction was achieved through direct impact on the optimal prescription of drugs and preventing the cost of drug side effects. In the end, the profit of this method was calculated using the difference in cost savings and salary paid to the clinical pharmacist.</span></span></span></span></span></span></span><br> <span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:12.0pt"><span style="page-break-after:auto"><span style="unicode-bidi:embed"><span style="font-size:10.0pt">Results</span><span style="font-size:10.0pt"><span style="font-weight:normal">: A total of 350 interventions were performed on 137 patients and 87 interventions reduced the incidence of drug side effects. The presence of the clinical pharmacist in the intensive care unit economically led to a net benefit of 10-17 billion riyals during this period.</span></span></span></span></span></span></span><br> <span style="text-justify:kashida"><span style="text-kashida:0%"><span style="line-height:12.0pt"><span style="page-break-after:auto"><span style="unicode-bidi:embed"><span style="font-size:10.0pt">Conclusion</span><span style="font-size:10.0pt"><span style="font-weight:normal">: Interventions of clinical pharmacists in the intensive care unit not only help to improve the clinical results of patients, but can also lead to reducing treatment costs and increasing the quality of services provided.
