fa خانه بهداشت الگوی اقتصاد مقاومتی در نظام سلامت کشور تخصصي Special مروري Review Article <strong><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;">زمینه و هدف:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;"> خانه بهداشت محیطی‌ترین واحد ارائه خدمات سلامت در مناطق روستایی کشور است. این واحد ارایه خدمت با فلسفه گسترش عدالت در سلامت و بهره‌مندی از خدمات سلامت تشکیل شد. در این مطالعه هزینه فعالیت‌های خانه بهداشت در سال 1374 برای جلوگیری از مرگ و معلولیت یک کودک زیر 5 سال با هزینه درمانی اعمال جراحی مقایسه می‌شود.</span></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;">روش:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;"> اطلاعات مورد نیاز مطالعه به روش بررسی متون از مستندات موجود و نتایج مطالعات انجام شده در خصوص هزینه‌های خانه بهداشت جمع‌آوری شد. همچنین اطلاعات موجود در درگاه‌های ملی مرکز آمار ایران، سازمان ثبت احوال، بانک مرکزی، اطلاعات وزارت بهداشت درمان و آموزش پزشکی و بالاخره مصاحبه با افراد دست‌اندرکار توسعه خانه‌های بهداشت حاصل و سپس هزینه جلوگیری از مرگ و معلولیت کودکان زیر 5 سال محاسبه شده است.</span></span><br> <strong><span style="letter-spacing:0pt;"><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;">یافته‌ها:</span></span></span></strong><span style="letter-spacing:0pt;"><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;"> هزینه بهداشت یک نفر روستایی در سال 1374 برابر 10 دلار در آن سال بوده است. بنابراین 3 دلار در سال برای هر کودک هزینه می‌شود تعداد مرگ‌های کاهش یافته در سال 1374 برابر 1200 نفر براوردگردید و تعداد کودکانی که از معلولیت آن‌ها جلوگیری شده است نیز برابر 1200 نفر محاسبه شده است. بدین ترتیب، در سال 1374 از 2400 مرگ یا معلولیت کودک جلوگیری شده است. با احتساب جمعیت زیر 5 سال؛ مراقبت‌های بهداشتی درمانی اولیه در مقابل هزینه به‌طور تقریبی 3000 دلار در سال برای هر کودک زیر 5 سال توانسته است یک کودک را از مرگ و یا ناتوانی نجات دهد. در</span></span></span> <span style="letter-spacing:0pt;"><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;">مقابل هزینه یک درمان پر هزینه مثل عمل قلب باز یا پیوند کلیه یا پیوند کبد درآن سال حداقل 000</span></span></span><span dir="LTR" style="letter-spacing:0pt;">,</span><span style="letter-spacing:0pt;"><span style="font-family:B Nazanin;"><span style="font-size:11.0pt;">10 دلار هزینه داشته است.</span></span></span><br> <strong><span dir="RTL" style="letter-spacing:-.3pt;"><span style="font-family:Times New Roman,serif;"><span style="font-size:12.0pt;">نتیجه‌گیری:</span></span></span></strong><span dir="RTL" style="letter-spacing:-.3pt;"><span style="font-family:Times New Roman,serif;"><span style="font-size:12.0pt;"> براساس نتایج به‌دست آمده و بررسی مستندات موجود نشان داده شد که در سال 1374 هزینه اثربخشی مراقبت‌های بهداشتی درمانی اولیه در مقایسه با اعمال جراحی بزرگ مثل عمل قلب باز یا پیوند کلیه از لحاظ هزینه و طول عمر اضافه شده برابر 18 است. به عبارت دیگر هزینه اضافه کردن یکسال عمر در اعمال پیشرفته 18 برابر هزینه اضافه کردن یک سال عمر در برنامه مراقبت‌های بهداشتی درمانی اولیه برای کودکان زیر 5 سال است.</span></span></span> <div style="text-align: justify;"><strong>Background</strong>: The Iranian Health House is a peripheral unit of the Iranian Primary Health care (PHC); it is the most comprehensive health service unit in rural areas of the country. These Healthcare units, were formed with the intention of increasing of social and health equity in health to benefit those in need of healthcare services. In this study, the cost of health services delivered in 1995 to prevent child mortality and disability in children under 5 years of age were assessed.<br> <strong>Methods:</strong> Data were obtained by literature review of studies conducted on the costs of Healthcare clinics as well as information from websites of the Statistics Center of Iran, National Organization for Civil Registration, Central Bank, Ministry of Health and Medical Education and interviews with key informant involved in the development of healthcare centers and then the cost of preventing death and disability of children under 5 years of age were estimated.<br> <strong>Results:</strong> The healthcare cost of a villager was US$ 10 per year in 1995. Health expenditure of a child was US$ 3 per year. The number of deaths and disabilities of children prevented were reduced was 2400 in 1995. Taking into account the population under 5 years, primary health care costed about US$ 3,000 a year for each child under 5 saved from death or disability.
